By submitting a reservation with Pampered Pets of Paducah, Inc. you are agreeing to abide by the Policies of Pampered Pets of Paducah, Inc.

1. Reservations are required (Emergencies - please call or bring pet to facility during Drop-Off

Session hours)

2. Drop-Off and Pick-up times MUST be observed

3. All pets must have shots up to date including Bordatella and HeartGard for dogs

4. All pets must be on a flea and tick program

5. All pets need to bring their own food, treats, toys, and medications.

(We provide bowls, bedding, and additional toys with individualized care)

6. All pets must have a current (< one year), up-to-date Pet Profile on file with Pampered Pets

7. I give my permission for Pampered Pets of Paducah, Inc. to seek veterinarian assistance in case of

sickness or emergency. I understand any veterinarian charges are my complete and sole

responsibility.

8. There will be a $2 Service Fee if your Pampered Pet Reservation form and/or Pay Pal receipt are not

provided at Check In.

After you choose a date & time, you will create an account with your details and complete the booking.

Your email address and password will then allow you to review, change and add appointments.
